Job summary

Chinese Community Social Services Centre in Box Hill, VIC seeks a Full-time Care Partner to join the Support at Home team. Based at our Head Office, you will coordinate tailored aged care solutions for older people to live safely at home, managing a diverse caseload and collaborating with families, care workers, and providers.

You will develop person-centred care plans, monitor wellbeing, manage budgets and contribute to service development.

Qualifications

• Certificate III or IV in Community Services or equivalent required.
• Experience in aged care, disability, or community services preferred.
• Australian Police Check and right to work in Australia required.

Responsibilities

• Manage a caseload of older adults and coordinate care plans.
• Develop and review person-centred care plans and budgets.
• Monitor wellbeing, identify risks, and refer to services as needed.
• Coordinate services with families, providers and internal teams.
• Maintain documentation and support continuous improvement.
