BlueCare, part of the UnitingCare family, is seeking a Palliative Care Officer to support in-home palliative services across Caboolture with hybrid working arrangements. The role reports to the Palliative Care Manager and involves coordinating referrals, scheduling services, and acting as the first point of contact for clients and carers.

Formación

• Certificate III or higher in Health Admin, Community Service or equivalent.
• Experience liaising with clinical staff and coordinating care.
• Ability to understand complex funding models and guidelines.

Responsabilidades

• Schedule all Palliative Care services and manage referrals promptly.
• Provide professional first point of contact to clients and carers.
• Respond to client enquiries and escalate as required to clinicians.
• Coordinate with palliative care consultants and nursing teams.
• Maintain effective communication with neighbourhoods and allied services.
