BlueCare, part of UnitingCare Queensland, is seeking a Palliative Care Officer to join our state-wide team. The role focuses on delivering in-home palliative care with a client-centered approach, coordinating services, and ensuring smooth referrals and communication with clinical staff.

The position offers a 12-month fixed-term, full-time basis with hybrid working arrangements at Caboolture/Alexandra Hill, attractive remuneration of $40.58–$43.52 per hour plus super and salary packaging benefits.

Formación

• Certificate III or higher in Health Administration or Community Service or equivalent experience.
• Experience connecting teams, delivering customer service and solving problems in a multidisciplinary setting.
• Ability to liaise with clinical staff to coordinate care and exercise professional judgment.
• Understanding complex funding models and compliance with government guidelines.
• Strong interpersonal and written communication skills; proficient computer use.

Responsabilidades

• Accept and triage internal and external referrals for Palliative Care services.
• Schedule all Palliative Care services in a timely manner.
• Provide first point of contact/concierge services to clients and carers.
• Respond to enquiries and escalate as required to the appropriate teams.
• Maintain effective communication and facilitate rapid referrals to health professionals.
• Engage with communities and regional services to identify client needs.
