Help In Support is seeking an experienced and motivated NDIS Support Coordinator to join our team on a subcontractor basis.


This opportunity would suit an experienced Support Coordinator who can confidently manage their own caseload while maintaining a strong focus on participant outcomes, safeguarding, compliance, and effective management of NDIS funding.


About the Role

As a Support Coordinator, you will work directly with NDIS participants to help them understand and implement their NDIS plans, effectively manage their funding and connect with appropriate services to achieve their individual goals.


Key responsibilities include:


  • Providing high-quality, person-centered Support Coordination

  • Supporting participants to understand their NDIS plans, funding and available supports

  • Monitoring and managing participant NDIS budgets across different support categories

  • Supporting participants to appropriately allocate funding across Core, Capacity Building, Capital and other relevant support categories

  • Monitoring expenditure and plan utilization to help ensure funding remains sustainable throughout the plan period

  • Identifying potential over-utilization or under-utilization of funding and taking appropriate action

  • Assisting participants to understand funding flexibility, stated supports and relevant funding restrictions

  • Working with participants, plan managers and providers to monitor expenditure and resolve funding or service-related issues

  • Connecting participants with suitable NDIS, mainstream and community supports

  • Coordinating services across complex support networks

  • Liaising with participants, families, nominees, guardians, allied health professionals, plan managers and service providers

  • Preparing progress reports, plan review reports and other NDIS documentation

  • Maintaining accurate, timely and professional case notes and records

  • Identifying and responding appropriately to participant risks, incidents and safeguarding concerns

  • Understanding and meeting relevant NDIS compliance and reporting requirements

  • Working in accordance with the NDIS Practice Standards and NDIS Code of Conduct

  • Maintaining appropriate professional boundaries, privacy and confidentiality

  • Supporting participant choice, control, dignity and independence

  • Escalating risks, incidents or safeguarding concerns appropriately and in a timely manner


About You

We are looking for someone who:



  • Has demonstrated experience working as an NDIS Support Coordinator

  • Has a strong understanding of NDIS plans, funding structures and support categories

  • Has demonstrated experience monitoring participant budgets, plan utilization and expenditure

  • Can identify funding risks and support participants to maximize their plan while maintaining appropriate budget sustainability

  • Understands the different NDIS support categories and how funding may be utilized in accordance with a participant's plan

  • Has a sound understanding of NDIS compliance requirements, safeguarding and risk management

  • Has a strong working knowledge of the NDS Code of Conduct

  • Understands incident management, reportable incidents, participant rights, privacy and duty of care

  • Can confidently manage their own caseload and competing priorities

  • Has strong written communication, documentation and report-writing skills

  • Can identify potential risks and elevate safeguarding or compliance concerns appropriately

  • Is reliable, organized, responsive and able to work independently

  • Demonstrates a strong commitment to participant choice and control

  • Can build effective professional relationships with participants, families and multidisciplinary stakeholders


Requirements

The successful applicant will be required to have:



  • Current ABN

  • Appropriate professional indemnity and public liability insurance

  • Current NDIS Worker Screening Check

  • Working with Children Check where applicable

  • Current National Police Check

  • Relevant qualifications and/or demonstrated experience in disability, community services or a related field

  • Own computer, phone and reliable internet access
