Job summary

Allambi Care is seeking a dedicated Support Coordinator in the NDIS department to assist participants in understanding and using their NDIS plans. You will link clients with providers, negotiate service agreements, manage budgets and support independence.

You will work closely with participants to tailor supports to goals and ensure compliance with NDIS standards, while fostering choice, safety and empowerment in planning and service delivery.

Qualifications

• Strong knowledge of the National Disability Insurance Scheme (NDIS) and coordination processes.
• Proven negotiation and advocacy skills with providers and participants.
• Experience identifying and managing risks, incidents, and complaints.

Responsibilities

• Coordinate supports to align with each participant's plan and goals.
• Link participants to mainstream, community and provider services.
• Negotiate service agreements and monitor budget usage.
• Ensure compliance with NDIS Practice Standards and Allambi Care policies.
• Maintain accurate records of coordination activities and communications.
