Job summary

The Star Care Group in Western Australia is seeking a Part-time Support Coordinator to assist participants in implementing their NDIS plans. You will connect participants with services, coordinate multi-disciplinary support, and help families navigate education, health, and community resources.

The role emphasizes safeguarding participant rights and promoting independence. Experience with NDIS standards and person-centred planning is preferred.

Qualifications

• NDIS awareness and understanding of participant rights.
• Experience with NDIS Practice Standards and related guidance.
• Knowledge of National Child Safe Standards and state requirements.
• Ability to work with participants who may be more vulnerable and manage risk.
• Strong report writing and stakeholder liaison skills.

Responsibilities

• Connect participants with relevant services and supports.
• Set up appointment bookings.
• Help participants understand and maximise their NDIS plan.
• Manage challenges in the support environment (education, health, justice).
• Reduce barriers to plan implementation and build capacity.
• Develop and design support solutions to meet participant outcomes.
• Assist in plan funding understanding and linkage to broader supports.
• Prepare comprehensive reports with stakeholders.
