Job summary

Shine Health Services, based in Springwood, Australia, is seeking a dedicated Support Coordinator to deliver high‑quality case management for participants under the NDIS framework. The role requires building strong relationships, coordinating budgets, and guiding participants toward their goals.

You will coordinate plans, manage agreements, and collaborate with a diverse team to ensure timely documentation and compliance, including weekly billing targets and participant confidentiality.

Qualifications

• Diploma level in Social Work, Community Services or Mental Health; degree preferred.
• Minimum 3 years of NDIS Support Coordination experience including psychosocial recovery coaching.
• Strong knowledge of NDIS funding, plan reviews, and documentation.
• Excellent written communication and time management skills.
• Valid NDIS Worker Screening Card, First Aid and CPR certifications.
• Driver's licence and own vehicle; able to travel for client visits.

Responsibilities

• Provide high‑quality Support Coordination to participants and build rapport.
• Deliver goal‑driven psychosocial recovery coaching to participants with mental health needs.
• Explain and manage participant agreements and NDIS budgets.
• Conduct plan reviews and update changes in circumstances.
• Research and vet allied health providers to ensure quality care.
• Maintain accurate documentation and weekly billing targets.
• Onboard new participants and assist with hospital discharges.
