Job summary

Shine Health Services, based in Springwood, is seeking a dedicated Support Coordinator. You will provide case management and psychosocial recovery coaching to participants, working to improve independence and outcomes within NDIS frameworks.

Role involves plan reviews, coordinating with providers, documenting changes, and maintaining confidentiality. This full-time position requires a local Queensland presence with on-site expectation at Springwood, plus offsite client visits.

Qualifications

• Relevant qualification in Social Work, Community Services or Mental Health at minimum Diploma level.
• Degree level is preferred.
• In-depth knowledge of NDIS and funding, plan reviews and documentation.
• Excellent written documentation and time management.
• Ability to work independently and with a diverse team.
• Commitment to confidentiality and professional boundaries.
• Valid NDIS Worker Screening Card, First Aid and CPR.

Responsibilities

• Provide high-quality case management to participants.
• Offer goal-driven psychosocial recovery coaching.
• Explain and manage participant agreements and NDIS budgets.
• Conduct plan reviews and manage changing circumstances.
• Vet allied health providers to ensure quality care.
• Build relationships with participants and stakeholders.
• Maintain timely documentation and weekly billing targets.
• Onboard participants and manage hospital discharges.
