Job summary

Phoenix Specialised Youth And Disability Services is seeking an experienced Level 2 Support Coordinator to support participants across NDIS, Child Protection, and Forensic Disability sectors in Victoria. The role requires proactive collaboration within a team and independent delivery of high-quality outcomes.

Qualifications

  • Experience working with people with disability.
  • Background in community services, disability, mental health, or related sectors.
  • Strong understanding of NDIS Practice Standards, Work Health and Safety, and Equal Opportunity principles.
  • Ability to problem-solve, show initiative, and achieve positive participant outcomes.
  • Knowledge of Aboriginal and Torres Strait Islander cultures (highly regarded).
  • Strong knowledge of local disability, mainstream, and community services, with proven networking ability.
  • Excellent organisational and time management skills.
  • High-level verbal and written communication skills.
  • Experience in person-centred planning and service delivery.
  • Sound computer literacy, including report writing and documentation.
  • Knowledge of the NDIS and relevant legislation, including the NDIS Act 2013 and Disability Act 2006.

Responsibilities

  • Manage and support a caseload of approximately 20 participants.
  • Develop and implement effective case plans.
  • Assist participants and families to understand and utilise their NDIS plans.
  • Facilitate care team meetings with participants and key stakeholders.
  • Build participant capacity and promote independence.
  • Monitor plan budgets and outcomes to ensure effective utilisation of funding.
  • Develop and maintain professional relationships while upholding appropriate boundaries.
  • Liaise with mainstream and community services (e.g. Housing, Education, Justice, Health) to ensure obligations are met.
  • Maintain accurate and compliant documentation, including case notes, reports, and billing records.
  • Ensure compliance with relevant legislation, standards, and internal frameworks.

Essential Requirements
  • Current Driver’s Licence

  • Valid Working with Children Check (Employee)

  • Current National Criminal History Check (within the last 6 months)

  • NDIS Worker Screening Check

  • Evidence of Australian working rights

  • Current First Aid and CPR certification

  • NDIS Orientation Certificate

  • Evidence of COVID-19 vaccination (including booster) or valid medical exemption
