Job summary

Blue Marble Health Services is seeking a dedicated NDIS Support Coordinator to empower participants to navigate their plans and achieve goals in a person-centred way. We offer a development pathway into Specialist Support Coordination, with mentoring and ongoing training, plus hybrid work across Brisbane, Gold Coast, Ipswich and surrounds.

Qualifications

• Experience in Support Coordination or a related support role
• Knowledge of the NDIS and a strong commitment to the rights and goals of people with disability
• Strong time management and communication skills
• Ability to work independently and as part of a team
• Capacity to travel within SE QLD (Brisbane, Gold Coast, Ipswich & surrounds)
• Valid Driver’s Licence and reliable, insured vehicle
• Current First Aid and CPR
• NDIS Worker Screening Check & National Criminal History Check (or willingness to obtain)

• Assisting clients to understand their funded supports and make informed choices
• Coordinating a network of supports, including providers, health professionals, and informal networks
• Advocating for participant needs and ensuring services align with their goals
• Keeping thorough documentation and maintaining compliance with NDIS requirements
• Identifying and managing risks, and supporting participants through plan reviews
• Staying current with NDIS practices and changes
• Participating in reflective practice, supervision, and team development activities

What You’ll Be Doing

What You Bring to the Role

• Capacity to manage a caseload and travel within Brisbane, Gold Coast, Ipswich & surrounding areas

• Valid Driver’s Licence and reliable, insured vehicle

• Current First Aid and CPR

• NDIS Worker Screening Check & National Criminal History Check (or willingness to obtain)
