This is a full-time position (38 hours per week) with flexible working arrangements.
The role is hybrid working in the office and from home with initial training being within the office, with flexibility to attend participant meetings and community appointments as required.
Provide Level 2 Support Coordination services to NDIS participants
Assist participants to understand and implement their NDIS plans
Build participant capacity to coordinate and manage their own supports
Develop and maintain strong relationships with participants, families, providers and key stakeholders
Coordinate services and resolve barriers to accessing supports
Advocate for participants while promoting choice, control and independence
Maintain accurate case notes, reports and participant records
Monitor participant progress towards goals and support positive outcomes
Work collaboratively with internal staff and external service providers
Minimum one year experience working as a support Coordinator within the NDIS
Strong understanding of the NDIS, relevant legislation and operational guidelines
Excellent organisational and time management skills
Exceptional communication, writing and relationship-building abilities
Ability to work independently while managing a varied caseload
Commitment to person-centred and strengths-based practice
Ability to work independently
Current Driver's Licence and reliable vehicle
NDIS Worker Screening Check
Working with Children Check (or willingness to obtain)
