Job summary

Power Connect Care Pty Ltd is seeking an experienced NDIS Support Coordinator in Sydney, NSW, for a casual, flexible role. You will assist participants in understanding and implementing their NDIS plans, linking them with appropriate supports and resources, and supporting them to achieve personal goals.

Qualifications

• Must have experience as an NDIS Support Coordinator.
• Strong written communication skills for reports and case notes.
• Ability to build relationships with participants, families and providers.
• Demonstrate organisational skills and ability to manage priorities.

Responsibilities

• Help participants understand and implement NDIS plans.
• Connect participants with services and resources.
• Maintain accurate participant records and documentation.
• Promote participant choice, Dignity and inclusion in service delivery.
• Collaborate with stakeholders to ensure services meet needs.
