Job summary

Circle of Hope Services in New South Wales, Sydney, seeks an experienced NDIS Support Coordinator to assist participants in understanding and implementing their plans, coordinating supports, and building capacity for independence.

You will work with participants, families, carers, and providers to achieve meaningful outcomes, maintain records, monitor progress, and ensure compliance with NDIS standards and child safety requirements.

Qualifications

• Knowledge and understanding of the NDIS, participant plan implementation, and support coordination practices.
• Person-centred, strengths-based, and recovery-oriented approaches.
• Strong organisational and time management skills with the ability to manage competing priorities.

Responsibilities

• Assist participants to understand and implement their NDIS plans.
• Coordinate and connect participants with appropriate supports.
• Build participant capacity to exercise choice, control, and independence.
• Support participants to achieve their identified goals and aspirations.
• Develop and maintain strong stakeholder relationships.
• Monitor service delivery and participant outcomes.
• Conduct plan reviews and prepare progress reports.
• Maintain accurate, timely, and compliant case notes and participant records.
• Identify risks and implement safeguarding measures.
• Ensure compliance with NDIS Practice Standards and Code of Conduct.
