Job summary

OnSide Support Coordination is seeking a dedicated Support Coordinator to assist clients under the NDIS across Bankstown and surrounding areas. You will coordinate services, manage plans, and help clients achieve their goals while working with a national service that has operated since 2017.

Roles are permanent with flexible work patterns including home visits, client sessions in the community, and office days to enable team collaboration.

Qualifications

• Strong relationship-building skills.
• Excellent organisational and time-management abilities.
• NDIS knowledge or willingness to learn.
• NDIS Worker Screening Check clearance required.

Responsibilities

• Coordinate supports across multiple providers.
• Support clients to gather resources and achieve plan goals.
• Manage funding budgets and track spending across plans.
• Document client information for reports and updates.
• Collaborate with stakeholders to deliver coordinated services.
