Job summary

Access Support Coordination in Gladstone, QLD is seeking a dedicated Support Coordinator to assist participants in navigating their NDIS journeys. You will help them understand options, make informed decisions and maximise funding value, all while building trust and supporting each person’s goals.

You will work across disability services, collaborating with families and professionals to coordinate plans, gather evidence and identify opportunities in housing, education and employment, ensuring

Qualifications

• Proven experience in a Support Coordination role within the disability sector.
• Qualifications in Disability, Community Services, Social Work or a related field.
• Strong knowledge of the NDIS and its processes.
• Ability to manage KPIs and billable hour targets.

Responsibilities

• Support participants to build meaningful, connected lives beyond their funded supports.
• Help participants understand and get the most out of their NDIS plans.
• Coordinate supports efficiently to maximise outcomes and funding utilisation.
• Gather evidence reflecting participants' evolving needs and future funding requirements.
• Identify opportunities across employment, recreation, education and housing.
• Advocate for participants and liaise with mainstream services to ensure the right supports are in place.
