Aligned Community Care is seeking a Clinical Engagement and Development Coordinator to join our Austral office. You will manage the end-to-end intake pipeline, connect participants with supports, and build strong referral networks across Sydney and Wollongong.

You will work closely with the head of operations and growth team, ensure timely, audit-ready documentation, and maintain confidentiality while expanding partnerships with allied health professionals and providers.

Qualifications

• Tertiary qualification in Occupational Therapy, Social Work, or related allied health field.
• Solid knowledge of the National Disability Insurance Scheme (NDIS).
• Strong person-centred practice.
• Excellent communication and interpersonal skills.
• Strong stakeholder engagement and relationship management.
• Excellent attention to detail and accuracy.
• Commitment to confidentiality, professionalism and ethical practice.

Responsabilités

• Manage end-to-end intake pipeline from initial enquiry to onboarding.
• Explain services, funding pathways to participants and families.
• Build and maintain referral relationships with allied health professionals.
• Complete audit-ready intake documentation and maintain accurate records.
• Coordinate with internal teams to match participants with supports.
• Lead external stakeholder correspondence on behalf of the organisation.
• Track referral sources and report on pipeline performance.
• Coordinate cross-department collaboration and ensure alignment with business needs.
• Liaise with external agencies while upholding privacy.
