Job summary

Sirv in Geelong, Victoria, is seeking a dedicated NDIS Home Team Leader to oversee a team of support workers delivering in-home disability services under the NDIS.

You will manage day-to-day operations, coordinate client assessments and care plans, ensure quality standards, and collaborate with therapists and healthcare providers.

The role requires strong leadership, communication, rostering, and a solid understanding of NDIS guidelines.

Qualifications

• NDIS knowledge and regulatory understanding.
• Experience in a leadership role within disability or health care.
• Current First Aid and CPR certifications.
• Ability to roster, evaluate performance, and develop staff.
• Strong interpersonal skills with clients, families, and stakeholders.
• Familiarity with Australian disability support legislation and standards.

Responsibilities

• Lead a team of support workers with guidance, supervision, and ongoing training.
• Ensure staff comply with policies, procedures, and quality standards.
• Oversee client intake, assessments, care plans, goal setting, and progress reviews.
• Monitor service delivery to meet individual client needs.
• Collaborate with therapists or healthcare providers involved in client care.
• Manage staff rostering to ensure adequate coverage within budget.
• Conduct regular performance evaluations with constructive feedback.
• Maintain accurate records and documentation per guidelines.
