Curamoir Healthcare Recruitment – Gregory Hills NSW is seeking an experienced Care Coordination Lead to manage a caseload of NDIS participants. You will ensure care plans, risk documentation and service delivery stay current and aligned with individual goals.

You will partner with intake, rostering and practice teams to support onboarding, continuity of care and timely responses to change. The role includes coaching and supervising support workers, with escalation of safeguarding and high-risk

Qualificações

• Qualification in disability, allied health, social science or equivalent.
• Experience in care coordination within disability or community services.
• Prior experience leading / managing support workers.
• Knowledge of NDIS processes, documentation, incidents and safeguarding.
• Strong communication and organisation skills.
• Current driver licence and ability to do home visits.

Responsabilidades

• Coordinate safe, responsive supports for an allocated caseload of NDIS participants.
• Collaborate with intake, rostering and practice/compliance stakeholders to support onboarding and continuity of care.
• Provide day-to-day leadership to allocated support workers and business staff, including coaching and supervision.
• Escalate safeguarding concerns and high-risk matters through internal pathways.
• Attend the office on a full-time basis.
