Job summary

Curamoir Healthcare Recruitment is recruiting for a full-time disability care coordinator based in Gregory Hills, NSW. You will coordinate supports for an allocated NDIS caseload, maintaining care plans, risk records and service delivery aligned to individual goals.

You will partner with intake, rostering and practice teams to ensure smooth onboarding and timely responses to changes, while coaching and supervising support staff and escalating high-risk matters as needed.

Qualifications

• Qualified in disability, allied health, social science, welfare or equivalent.
• Experience in care coordination/case management within disability or community services.
• Knowledge of the NDIS, documentation, incidents and safeguarding.

Responsibilities

• Coordinate safe, responsive supports for an allocated caseload of NDIS participants.
• Ensure care plans and risk documentation and service delivery remain current and aligned to goals.
• Lead and supervise allocated support workers and business staff, including coaching and training.
• Escalate incidents and safeguarding concerns through internal pathways.

The role:

You will coordinate safe, responsive supports for an allocated caseload of NDIS participants, ensuring care plans, risk documentation and service delivery remain current, accurate and aligned to individual goals.

You will work closely with intake, rostering and practice/compliance stakeholders to support smooth onboarding, continuity of care, and timely responses to changes in needs. You will also provide day-to-day leadership to allocated support workers and business support staff, including coaching, supervision and follow-up on training and competency requirements, while escalating incidents, safeguarding concerns, restrictive-practice issues and other high-risk matters through the appropriate internal pathways.

Requirements:

• Qualification in disability, allied health, social science, welfare, community services (or equivalent relevant experience)
• Experience in care coordination/case management within disability or community services
• Working knowledge of the NDIS, documentation, incidents and safeguarding
• Strong communication, organisation and systems capability (CRM/MS Office)
• Current driver licence; able to do home visits as required
• Current checks as required (NDIS Worker Screening, WWCC, police check)
