Job summary

Curamoir Healthcare Recruitment PTY is seeking a dedicated NDIS Support Coordinator to manage an allocated caseload of participants. You will build strong relationships with participants and families to understand goals, needs and preferences, and connect them with funded, mainstream, community and informal supports.

You will coordinate services across providers, review plans and budgets, and prepare reports while ensuring safeguarding and compliance.

Qualifications

• Qualification in disability, allied health, social science, welfare, or equivalent relevant experience.
• Experience in coordinating supports within disability or community services.
• Working knowledge of the NDIS, documentation, incidents and safeguarding.
• Strong communication, organisation and systems capability (CRM/MS Office).
• Current driver licence; able to do home visits as required.
• Current checks as required (NDIS Worker Screening, WWCC, police check).

Responsibilities

• Coordinate safe, responsive and person-centred supports for an allocated caseload of NDIS participants, building strong relationships with participants and families.
• Coordinate and monitor services across providers and stakeholders to align with participant goals.
• Review Care Plans, Risk Assessments and Service Agreements, and support plan reviews.
• Maintain accurate case notes and documentation, ensuring safeguarding compliance and appropriate escalation.
