Job summary

Proveda in Sydney, NSW is seeking a Care Coordinator with experience in the NDIS and/or Aged Care sectors to help people navigate the care system and plan supports. You will provide impartial information, support families, and liaise with providers to coordinate services.

Key requirements include Cert III or higher in community services or related fields, strong communication and digital skills, and compliance checks. Flexible working arrangements and staff benefits are offered.

• Cert III or above in community services, disability, social work or related discipline.

• Knowledge of service delivery systems incl. NDIS, MAC, The Lookout Way, NSW Health or NGO service provision.

Responsibilities

• Provide impartial information, guidance and navigation to the NDIS, My Aged Care and health services for customers and families.
• Support customers to set and achieve goals within their plan or package budget via phone and visits.
• Connect customers with informal, mainstream and funded supports.
• Liaise with a network of approved providers to ensure care and advocate for changes in support when needed.
• Support customers and families to build capability to manage their own care.

Proveda is one of the largest community-led organisations based in the Northern Beaches, providing independent and impartial information, advice and guidance services to older people, those with dementia, people with disabilities, severe and persistent mental illness as well as those with complex health needs. We work with people to successfully navigate the care and support system, implement their NDIS & Home Care Plan and to enhance their ability to access the support/s they require.

About the Role

Reporting to the Practice Managers, the Care Coordinator will have experience with either the NDIS, Aged Care or both.

The Care Coordinator facilitates a seamless experience for customers by

Providing impartial information, advice and guidance to people and their families/carers to navigate the NDIS, My Aged Care and the health services system.

Supporting customers to set and achieve their goals within their plan or package budget via phone and occasional customer visits.

Connecting customers with informal, mainstream and funded supports.

Building capacity of customers and their families/carers to manage their own care

The Care Coordinator is also liaising with a network of approved providers to ensure the desired and necessary care and support is provided to the client and advocate for changes in support and services to occur when a clear and persistent issue arises.

The salary range for the Care Coordinator role is $88,243.00 to $102,135.00 will be determined based on the successful candidate's skills, relevant sector experience, and overall knowledge.

About You

Cert III or above qualifications in community services, disability, social work, mental health or related discipline

Experience in NDISand/or Aged Care sectors

Demonstrated comprehensive knowledge of, and experience in service delivery systems including the NDIS, MAC, The Lookout Way, NSW Health or Community NGO service provision. (Preferrable)

Commitment to person centred and recovery approaches to care

Excellent communication skills both written and verbal

Excellent time management, multi tasking and prioritisation skills

Current NDIS worker Screening check & WWCC (or the ability to obtain)

Digital Skills including Client record management and Microsoft Suite
