Job summary

Curamoir Healthcare Recruitment in Sydney, NSW is seeking a dedicated coordinator to support NDIS participants. You will coordinate safe, person-centred supports, build relationships with families, assist participants with understanding and using their NDIS plans, and connect with funded, mainstream, and community supports.

You will monitor services across providers, review plans and budgets, document outcomes, and escalate risks as needed to ensure continuity of care.

Qualifications

• Experience with coordinating supports for NDIS participants.
• Strong written and verbal communication.
• Ability to manage multiple tasks and stakeholder relationships.
• Proficiency with CRM and Microsoft Office.
• Current driver licence and willingness to do home visits.

Responsibilities

• Coordinate safe, person-centred supports for an allocated caseload of NDIS participants.
• Monitor services across providers to align with participant goals and needs.
• Review Care Plans, Risk Assessments and Service Agreements, support plan reviews.
• Prepare reports, monitor budgets and advocate for appropriate supports.
• Maintain case notes, ensure documentation accuracy and safeguarding compliance.
