Inclusive Support Service is looking for a caring, organised and experienced NDIS Care Coordinator to join our team on a part-time basis (fixed term until September 2027) with possibility of extension.
About the Role
As an NDIS Care Coordinator, you will work closely with participants, families, support workers and other professionals to ensure participants receive the right support to achieve their goals.
In addition to coordinating participant supports, you will play a key leadership role within the organisation by supporting, mentoring and developing Team Leaders and House Coordinators. You will provide oversight across service sites, assist with quality and compliance processes, and contribute to continuous improvement initiatives. This role works closely with the management team and actively participates in leadership and head office meetings to help ensure high-quality, person-centred service delivery across the organisation.
What You'll Be Doing
- Develop a roster for NDIS clients with guidance from support coordinators, keeping in mind client choice and control.
- Coordinate Team Leader meetings, participant team meetings and other operational meetings as required.
- Undertake active shifts with clients.
- Work collaboratively with participants, families and support networks.
- Supervise, lead, mentor and support Team Leaders and House Coordinators to build capability, confidence and leadership skills.
- Identify training and development opportunities and assist in upskilling staff across service areas.
- Provide operational oversight of disability accommodation and community participation services to ensure quality supports are delivered.
- Conduct regular site visits and audits to ensure services, documentation and environments meet organisational, legislative and NDIS standards.
- Develop and maintain strong relationships with support workers and external providers.
- Monitor participant goals, needs and changing circumstances.
- Review participant and staff documentation to ensure accuracy, compliance and quality outcomes.
- Assist in the development of Support Plans, client documentation and review reports.
- Coordinate appointments, services and referrals.
- Communicate with allied health professionals and other stakeholders.
- Participate in the on-call roster on a weekly rotation.
- Monitor incident reports, identify trends and support follow-up actions to improve participant outcomes.
- Assist with incident investigations, documentation and reporting requirements where required.
- Support participant transitions, including new participant onboarding, changes in accommodation and/or support arrangements.
About You
We're looking for someone who is:
- Experienced in NDIS coordination, support coordination, case management or a similar role.
- Knowledgeable about the NDIS and participant plans.
- Highly organised with excellent communication skills.
- Confident working independently and as part of a team.
- Compassionate, approachable and person-centred.
- Comfortable communicating with families, support workers and professionals.
- Able to manage multiple participants and competing priorities.
- Passionate about supporting people with disability to achieve their goals.
Qualifications & Requirements
- Relevant qualification in Community Services, Disability, Social Work, Case Management or a related field.
- Previous NDIS experience highly desirable.
- Current NDIS Worker Screening Check.
- Current Working with Children Check (Blue Card), where applicable.
- Current First Aid & CPR.
- Current Australian driver's licence and reliable vehicle.
