Job summary

ATSICHS Brisbane is seeking a skilled practitioner to deliver kin carer case management in a permanent full-time role based in Brisbane. You will engage extended family members, assess eligibility, and coordinate care using established frameworks to support children and families in our Aboriginal and Torres Strait Islander community.

Qualifications

• Bachelor's degree in a relevant social services field with at least 4 years case management experience in family support, child protection, youth support, housing or related field.
• Diploma in relevant Community/Services field with at least 6 years in a case management role in family support, child protection, youth support, housing or related field.
• At least 6 years case management experience in family support, child protection, youth, housing or related field with commitment to Diploma while working and proof of Aboriginal and/or Torres Strait Islander status (or non-indigenous with organisation experience).

Responsibilities

• Engage clients (extended family and kin of child) through cold-call home-based visits and turn engagement into kin carer interest.
• Identify other strong family members during engagements and collect contact details and connections.
• Facilitate family placement discussions and assess families' views using NAT tools.
• Assist in carer assessments, Blue Card submission, and PAC assessments; support families in external assessments.
• Develop a Transition Plan addressing referrals, program tools, NATs, and assessments.
• Deliver case-managed services per the program model; maintain minimum caseload for equitable access.
• Maintain accurate case notes written from clients' voice and ensure client feedback is acted on.
• Participate in MOBnet supervision and complete monthly supervision tasks.
