Job summary

Anglicare NT is seeking a Case Manager to support young people transitioning from Out of Home Care into supported independent accommodation. You will deliver intensive case management, advocacy, outreach, life-skills development, and accommodation support.

Working within a trauma-informed, culturally safe, strengths-based framework, you will help build skills, confidence and networks to thrive independently.

Qualifications

• Qualifications in Human Services, Social Sciences or a related discipline.
• Experience supporting vulnerable or high-risk young people and families in community-based settings.
• Strong knowledge of the OOHC system and youth homelessness service sector.
• Demonstrated experience applying trauma-informed, strengths-based case management practices.
• Excellent engagement skills and the ability to work effectively with young people experiencing complex need.
• A commitment to culturally safe and inclusive practice, particularly when working with Aboriginal and Torres Strait Islander peoples and culturally diverse communities.
