Job summary

Anglicare NT is seeking a Case Manager to support young people aged 10 to 25 years who are homeless or at risk. You will provide intensive outreach, case management and practical support to help secure safe housing and strengthen family relationships.

You will work within a trauma-informed, strengths-based framework, with a focus on culturally safe practice for Aboriginal and Torres Strait Islander peoples and diverse communities.

Qualifications

• Qualifications in Human Services, Social Sciences or related discipline.
• Minimum four years’ experience working with vulnerable or high-risk young people and families.
• Trauma informed, strengths-based and case management practice.
• Knowledge of youth homelessness, Out of Home Care, youth justice and health systems.
• Strong engagement, assessment, advocacy and crisis intervention skills.
• Commitment to culturally safe and inclusive practice, particularly with Aboriginal and Torres Strait Islander peoples and culturally diverse communities.

Responsibilities
