Job summary

Anglicare NT in Darwin is seeking a YHOPP Case Manager to provide intensive, trauma-informed and youth-focused case management for young people aged 10 to 25 who are experiencing or at risk of homelessness.

Working within a strengths-based, culturally safe framework, you will help clients access safe accommodation, build independent living skills, and connect with services through outreach, assessment and collaborative planning. This is a short-term 12-month, part-time role (3 days per week).

Qualifications

• Qualifications in Human Services, Social Sciences or a related discipline
• Experience supporting vulnerable or high-risk young people and families
• Knowledge of the youth homelessness, out-of-home care, youth justice and health service systems
• Demonstrated experience applying trauma-informed, strengths-based case management practices
• Excellent engagement skills and the ability to work effectively with young people experiencing complex need
• A commitment to culturally safe and inclusive practice, particularly when working with Aboriginal and Torres Strait Islander peoples and culturally diverse communities
