Anglicare NT is seeking a dedicated Case Manager to support young people aged 10 to 25 years across the Katherine region. You will provide intensive outreach, case management and practical support to help access safe housing and strengthen family relationships.
This role requires trauma-informed, strengths-based practice and knowledge of youth pathways through homelessness, care, youth justice and health systems.

As Case Manager, you will work with young people aged 10 to 25 years who are experiencing or at risk of homelessness, including young people transitioning from Out of Home Care, youth justice and health settings. You will provide intensive outreach, case management, advocacy and practical support to help young people access safe housing, strengthen family relationships, build independent living skills and achieve sustainable outcomes

Experience of working closely with and having a strong understanding of Aboriginal & Torres Strait Islander culture would be considered an advantage.
