Job summary

You will develop case plans, collaborate with stakeholders, and document case notes while adhering to safeguarding and program requirements. This is a role for a community-minded professional.

Qualifications

• Experience delivering housing support services.
• Ability to engage with, and manage complex situations involving homelessness, poverty, mental health, family violence, alcohol and other drug misuse.
• Demonstrated cultural competency, particularly in working with Indigenous people.
• Demonstrated ability to advocate for clients and work effectively with stakeholders through meetings, letters of advocacy / support and case note writing.
• Valid Driver's licence (C Class)

Responsibilities

• Coordinating support to people who need to access immediate housing as well as those transitioning out of public housing.
• Assisting individuals, families and communities through prioritising referrals.
• Developing case plans and strategies to address challenges.
• Participating and collaborating in relevant networks with key stakeholders.
• Writing accurate case notes providing reports and feedback.
