Job summary

Institute for Urban Indigenous Health Ltd is seeking a Support Worker – Staying Deadly Hub in Caboolture to walk alongside clients through holistic care. You will collaborate with a multidisciplinary team to support Indigenous communities, promoting independence and improving access to services while respecting cultural needs.

The role emphasises working with Aboriginal and Torres Strait Islander peoples, including outreach in local communities and home visits to support client recovery.

Qualifications

• Aboriginal or Torres Strait Islander person is essential.
• Current Criminal History Check.
• Current C Class Drivers Licence (QLD).
• Current Working with Children's Check (Qld Blue Card).
• Experience in mental health, drug and alcohol services, or related social service fields.

Responsibilities

• Provide practical psychosocial supports.
• Promote client independence by developing and working towards individual goals.
• Remove barriers so clients have improved access to services and wellbeing.
• Work from a client and family-centred perspective with cultural considerations.
• Establish relationships with internal and external services to facilitate referrals and clear communication.
• Conduct home visits or community visits to support ongoing client recovery.
