Job summary

IUIH Institute for Urban Indigenous Health in Caboolture is seeking a Care Coordinator to support Aboriginal and Torres Strait Islander clients with health and social needs. You will work within a multidisciplinary team to deliver holistic care, wrap-around services, and intensive case management aimed at improving community wellbeing.

The role emphasizes trauma-informed practice, client empowerment, and coordination across health and social care services within our Staying Deadly Hub.

Qualifications

• Aboriginal or Torres Strait Islander person is essential for the role.
• Current Criminal History Check is mandatory.
• Valid Australian driver’s licence (QLD) is required.
• Working with Children Check (QLD Blue Card) must be current.
• CPR and First Aid certification are mandatory.
• Qualificação in Aboriginal and/or Torres Strait health or willingness to complete is required.
• Experience in mental health, drug and alcohol, or related social services is preferred.
• Degree or credential in Social Work/Mental Health/Behavioural Science or related field is required.

Responsibilities

• Develop and maintain support plans with clients and service providers.
• Apply trauma-informed, recovery/strengths-based approaches to wellbeing plans and safety assessments.
• Coordinate and deliver services for clients with mental health, drug/alcohol, chronic disease, and social health needs.
• Conduct home, community, and custody visits to support client recovery.
• Contribute to smooth transitions for clients accessing other services.

Education

Current Criminal History Check

Current C Class Drivers Licence (QLD)

Current Working with Children’s Check (Qld Blue Card)

Current CPR and First Aid Certificate

Certificate IV or Higher in Aboriginal and/or Torres Strait Primary Health or willing to complete

Experience in mental health, drug and alcohol services, or related social service fields

Qualification in Social Work/Mental Health/Behavioural Science or related field
