Job summary

IUIH Institute for Urban Indigenous Health is seeking a Staying Deadly Support Worker for the Caboolture area to walk alongside clients with holistic psychosocial supports. The role works with a multidisciplinary team to promote independence and reduce barriers to care within the community.

The role emphasizes cultural safety and is anchored in client and family-centred care, contributing to improved health outcomes for Indigenous communities in the Caboolture region.

Qualifications

• Experience in mental health, drug and alcohol services or related social services.
• Current Criminal History Check.
• Current C Class Drivers Licence (QLD).
• Current Working with Children’s Check (Qld Blue Card).

Responsibilities

• Promote client independence by developing and working towards individual goals.
• Help remove barriers to access services in the community.
• Provide client- and family-centred support respecting cultural and emotional needs.
• Coordinate with internal and external services to facilitate referrals and clear communication.
• Conduct home or community visits to support ongoing client recovery.

About the role

As Support Worker – Staying Deadly Hub, your role is to take lead from the care coordinator and multidisciplinary care team to holistically walk alongside our clients. Providing practical psychosocial supports in the following domains:

• Promoting client independence by developing and working towards individual goals.
• Working to remove barriers so that clients have improved access to services within the community that enhance individuals' emotional well-being.
• Working from a client and family centred perspective with consideration given to cultural, emotional, and physical needs.
• Working relationships with internal and external services to facilitate referral pathways and clear communication.
• Conduct home visits or community visits to support ongoing client recovery.
• Ensure engagement of natural supports and networks to compliment funded services and assist in supported decision making where relevant and consistent with customer preferences.
