Job summary
Moreton ATSICHS in Caboolture is seeking a Care Coordinator – Staying Deadly Hub to deliver outcome-focused health, social and practical support to Aboriginal and Torres Strait Islander clients. You will work within a multidisciplinary team to assist those with mental health needs in the community and support transitions from inpatient care to ongoing wrap-around services.
The role emphasizes trauma-informed practice, strength-based strategies, and comprehensive assessment of needs with safety
Qualifications
- Position restricted to Aboriginal or Torres Strait Islander applicants.
- Must have a Criminal History Check and Blue Card.
- Experience in mental health or social services preferred.
Responsibilities
- Develop and maintain support plans with clients and providers.
- Use trauma-informed, recovery/strengths-based approaches for wellbeing plans.
- Coordinate services for clients with mental health, drug/alcohol and chronic disease needs.
- Conduct home, community or custody visits to support recovery.
- Assist in smooth transitions to other services.
