Job summary

Institute for Urban Indigenous Health is seeking a Care Coordinator for the Staying Deadly Hub to deliver outcome-focused health, social and practical support to Aboriginal and Torres Strait Islander clients. You will work within a multidisciplinary team to support individuals with mental health needs and help them stay connected to community-based care.

Your day-to-day includes developing support plans, coordinating services, and conducting visits to support recovery and safe transitions across

Qualifications

• Qualification in Social Work/Mental Health/Behavioural Science or related field.
• Certificate IV or higher in Aboriginal and/or Torres Strait Primary Health or willingness to complete.
• Experience in mental health, drug and alcohol services, or related social services.

Responsibilities

• Develop and maintain support plans with clients and service providers.
• Use trauma-informed, recovery or strengths-based approaches to wellbeing planning.
• Coordinate services for clients with mental health, drug and alcohol or chronic disease needs.
• Conduct home or community visits to support client recovery.
• Contribute to smooth transitions when accessing other services.

Education

Certificate IV or Higher in Aboriginal and/or Torres Strait Primary Health

Social Work / Mental Health / Behavioural Science qualification or related field

Experience in mental health, drug and alcohol or related social services

Tools

C Class Drivers Licence (QLD)

Working with Children’s Check (Qld Blue Card)

Current CPR and First Aid Certificate
