Job summary

IUIH is seeking a Support Worker – Staying Deadly Hub in Caboolture to work with Aboriginal and Torres Strait Islander communities alongside a multidisciplinary team. You will provide psychosocial supports, help clients pursue personalised goals, and promote independence.

Responsibilities include home visits, coordinating referrals, and creating culturally safe care plans to support recovery and wellbeing in the community.

Qualifications

  • Experience working in mental health, drug and alcohol services, or related social service fields.
  • Current Criminal History Check.
  • Current C Class Drivers Licence (QLD).
  • Current Working with Children’s Check (Qld Blue Card).

  • Removing barriers so clients have improved access to services within the community.
  • Working from a client and family centred perspective with cultural, emotional, and physical needs.
  • Maintaining referral pathways and clear communication with internal and external services.

About the role

As Support Worker – Staying Deadly Hub, your role is to take lead from the care coordinator and multidisciplinary care team to holistically walk alongside our clients. Providing practical psychosocial supports in the following domains:

  • Promoting client independence by developing and working towards individual goals.
  • Working to remove barriers so that clients have improved access to services within the community that enhance individuals’ emotional well-being.
  • Working from a client and family centred perspective with consideration given to cultural, emotional, and physical needs.
  • Working relationships with internal and external services to facilitate referral pathways and clear communication.
  • Conduct home visits or community visits to support ongoing client recovery.
  • Ensure engagement of natural supports and networks to compliment funded services and assist in supported decision making where relevant and consistent with customer preferences.
