The Salvation Army (SA) Property Trust atf Social Work in Shepparton, VIC, is seeking a Part-time Case Manager to support people experiencing homelessness. You will deliver person-centred case management, outreach, referrals and housing pathways in a community setting.

Reporting to the Team Leader, the role is on-site, 30.4 hours per week, with SCHADS Award Level 5 pay. A tertiary degree or diploma in Social Work or related field and a valid Working with Children Check are required.

Qualifications

• Tertiary degree or diploma in Social Work or related field.
• Experience in homelessness, housing or community services advised.
• Ability to assess clients, develop and review support plans.

Responsibilities

• Deliver strengths-based, person-centred case management to individuals and families at risk of homelessness.
• Conduct client assessments, manage referrals and provide proactive outreach.
• Develop and review case plans linking clients to housing and essential services.
• Advocate for clients and partner with external service providers to achieve outcomes.
• Maintain accurate records and meet funding requirements.
