Job summary

Life Without Barriers is seeking a Case Coordinator for the National Immigration Support Service (NISS) based in Parramatta, with hybrid work options. You will conduct thorough assessments to determine client eligibility and work with clients to create and regularly update personalised case plans, linking them to community supports and resources.

You will help ensure client safety through risk assessments and accurate record keeping, build strong relationships with clients and families using

Qualifications

• Tertiary qualifications in Human Services or related discipline (minimum Certificate IV with relevant experience).
• Experience in case coordination and developing case plans that support clients' independent living and ongoing eligibility for services.

Responsibilities

• Conduct thorough assessments to determine client eligibility
• Create and regularly update personalised case plans
• Assist clients with referrals to community supports and resources
