Uniting NSW.ACT is seeking a dedicated Support Coordinator to empower participants in decision-making, coordinate funding, and connect them with mainstream and community services. The role supports people with disabilities to build capacity, negotiate services, and establish service agreements with providers of their choice.

Qualifications

• Diploma or higher tertiary qualifications in disability/community services/social sciences/allied health or equivalent.
• Knowledge of the NDIS Code of Conduct and Practice Standards.
• Understanding of the NDIS Act 2013 and related rules.
• Sound understanding of complex needs of people with disabilities and families.
• Experience with person-centred approaches to build choice and control.
• Ability to establish partnerships with communities and organisations.
• Proficient in Microsoft Office and client database systems; strong report writing and time-management skills.

Responsibilities

• Explore options with participants to support informed choices.
• Help participants to use MyID, the NDIS Portal, and negotiate services and agreements.
• Negotiate services and prices for quotable supports as needed.
• Arrange assessments to determine funding requirements (e.g., complex home mods).
• Coordinate with Plan Managers on funding, payments and services.
• Link participants to mainstream/community services (housing, education, transport, health).
• Strengthen participants’ capacity to coordinate supports and participate in the community.
• Resolve issues and ensure understanding of service agreements.
