Job summary

Uniting NSW.ACT is seeking a Support Coordinator to help participants access and coordinate NDIS supports across multiple NSW office sites, including North Parramatta, Leichhardt, and Campbelltown. This role offers hybrid work with 2 days remote and 3 days in the office, starting at SCHADS Level 4.1 with salary packaging up to $18,550.

You will engage with participants to navigate funding, providers, and supports, using person-centred approaches to build independence and outcomes.

Qualifications

• Diploma or higher qualification in disability/community services or allied health.
• Knowledge of the NDIS Code of Conduct.
• Knowledge of the NDIS Practice Standards and the NDIS Act 2013 and rules.
• Experience with person-centred approaches that support people to build capacity for choice and control.

Responsibilities

• Explore mainstream, community, advocate, informal and NDIS provider options with participants.
• Support participants to establish MyID, navigate the NDIS Portal, and service agreements with providers.
• Negotiate services and prices for quotable supports.
• Arrange assessments to determine funding needs (e.g. complex home modifications).
• Liaise with Plan Managers on funding, support types, payments.
• Link participants to mainstream/community services.
• Strengthen participants’ capacity to coordinate supports and participate in the community.
• Resolve problems or issues arising and manage service changes.
