Job summary

Pearl Home Care Gold Coast is seeking an experienced Care Partner to coordinate Support at Home packages and manage a caseload from onboarding through ongoing review. You will set up services, craft care plans, manage budgets, and serve as the primary contact for clients and families.

The role is office-based in Hope Island with hybrid, work-from-home days. You’ll work with an in-house clinical team, not a head office, and enjoy opportunities for growth within a locally owned provider.

Qualifications

• 2 years’ experience coordinating Home Care Packages or Support at Home services.
• Ability to explain classifications, budgets and care management clearly to clients and families.
• Strong planning, organisation and time management across a full caseload.

Responsibilities

• Onboard new clients by reviewing My Aged Care referrals, initial assessment and setting up service agreements and care plans.
• Build and review care plans around client goals, updating as needs change.
• Manage quarterly budgets, explain client contributions and ensure funds are used appropriately.
• Coordinate services with scheduling, support workers, and allied health providers.
• Support short-term pathways and timely reassessments when needs change.
• Identify and escalate clinical concerns to the Clinical Care Manager or RN.
• Maintain regular client contact and accurate care management records.
