Job summary

Mounties Care Home care is seeking a dedicated Care Partner to join our team on a full-time basis at our Hope Island, QLD office. You will work with our clients to help them stay as independent as possible at home and in the community, coordinating care and services with a trusted team of Carers to ensure quality delivery.

Key duties include developing client care plans, preparing budgets, assisting access to My Aged Care, visiting clients in the community, and supporting potential client leads

Qualifications

• Minimum 2-3 years' experience in a similar role.
• Current clear National Police Check.
• Current First Aid Certificate, including CPR.
• Demonstrated knowledge within the Aged Care sector.
• Demonstrated care coordination skills and ability to work effectively with clients, family members and service providers
• Previous experience managing Home Care Packages.
• Experience navigating the My Aged Care Portal.
• Strong understanding of the Aged Care Quality Standards.
• Prior experience with CRM and electronic rostering systems.
• Effective verbal and written communication skills.
• Excellent time management skills, with the ability to multitask.
• Positive and professional can-do attitude.

Responsibilities

• Develop and coordinate client care plans.
• Prepare individual client budgets.
• Assist clients to access My Aged Care.
• Visit clients in the community.
• Liaise with clients, families and Healthcare providers.
• Manage confidentiality and records.
• Participate in after-hours on-call roster.
• Coordinate care delivery with carers and service providers.
