Job summary

Right at Home Gold Coast is expanding, seeking experienced Case Managers, Care Managers and Care Coordinators to join our Support at Home team across Pimpama and Bundall.

As a Care Partner, you will manage a client portfolio, coordinate day-to-day care and support needs, and ensure services are safe, personalised and responsive to changing needs, in a high-quality home care environment.

Qualifications

• Previous experience in Case Management, Care Management, Care Coordination or Community Aged Care.
• Nursing qualifications are highly valued.
• Experience managing a client portfolio and coordinating community-based services.
• Knowledge of the Australian aged care sector and Support at Home.
• Certificate III in Individual Support/Aged Care or a higher relevant qualification.
• Strong communication, organisational and problem-solving skills.
• Excellent documentation and computer skills.
• Ability to work independently and manage competing priorities.
• Current Australian Driver Licence.
• Current or willingness to obtain an Australian Federal Police Check.

Responsibilities

• Manage and coordinate a portfolio of clients and their services.
• Conduct client reviews, welfare checks and ongoing care monitoring.
• Assess client needs and develop, review and update care plans.
• Coordinate services with clients, families, Care Workers and external providers.
• Monitor service quality, client outcomes and changing care needs.
• Respond to client enquiries, feedback and complaints.
• Maintain accurate client records and ensure compliance with relevant legislation and organisational requirements.
• Support continuous improvement and the ongoing development of our Support at Home services.
