Job summary

Western Health in Melton is seeking a Psychosocial Support Worker to join the HOPE Team. You will provide brief therapeutic interventions for individuals at risk of suicide, collaborating with the broader community mental health team to support recovery.

The role focuses on outreach and clinic-based care, with a 12-week post-discharge support plan aimed at reducing re-attempts and deaths by suicide. The role requires crisis intervention skills, engagement with welfare agencies, and experience

Qualifications

• Solid understanding of crisis intervention and management.
• Experience engaging with welfare, health, and other support agencies to improve consumer outcomes.
• Experience working with culturally and linguistically diverse communities.
• Current Victorian Driver’s Licence and ability to use for transport.

Responsibilities

• Provide direct support to consumers, carers, and families.
• Collaborate with community agencies to ensure comprehensive care.
• Work within a multidisciplinary team to deliver safe, positive experiences for at‑risk consumers.
• Communicate effectively to support recovery of consumers and families.
