Job summary

Western Health, based in Melbourne, is seeking Psychosocial Support Workers to join the HOPE Team providing brief therapeutic interventions for individuals at risk of suicide. The role focuses on outreach and clinic-based supports to reduce re-attempts over a 12-week period, with a commitment to patient- and family-centred care.

The role requires crisis intervention skills, collaboration with welfare and health agencies, and experience with diverse communities.

Qualifications

• Solid understanding of crisis intervention and management, with the ability to escalate concerns when necessary.
• Strong ability to engage welfare, health, and support agencies to improve consumer outcomes.
• Experience working with culturally and linguistically diverse communities.
• Current Victorian Driver's Licence and ability to use it for transport.

Responsibilities

• Provide direct support to consumers, carers, and families.
• Collaborate with other community agencies to ensure comprehensive care.
• Work as part of a multidisciplinary team to deliver a safe experience for at-risk consumers.
• Communicate effectively with consumers, carers, and families to support recovery.
