Western Health in Melton is seeking a dedicated Psychosocial Support Worker to deliver brief therapeutic interventions for individuals at risk of suicide. The role supports the HOPE team with outreach and clinic-based care for up to 12 weeks, within a multidisciplinary community mental health framework.

You’ll work with consumers, carers and families, collaborate with partner agencies, and contribute to recovery and safety planning in a culturally diverse community.

Formación

• Solid understanding of crisis intervention and management.
• Engaging with welfare, health and support agencies to improve outcomes.
• Experience with culturally and linguistically diverse communities.

Responsabilidades

• Provide direct support to consumers, carers and families.
• Collaborate with community agencies to ensure comprehensive care.
• Work in a multidisciplinary team to deliver a positive and safe experience for at-risk individuals.
• Communicate effectively with consumers, carers and families to support recovery.
