Receive and process referrals into appropriate services
Conduct comprehensive home-based assessments and WHS screenings
Identify risks and escalat where required
Assist prospective clients to understand funding pathways
Develop tailored care plans aligned with the Aged Care Quality Standards
Collaborate with internal/external providers to support independence and wellbeing
Monitor quarterly budgets and support reassessments
Provide guidance and direction to service delivery staff
Complete accurate and timely client documentation
Ensure compliance with legislation, organisational policies and quality standards
Record hazards and escalat risks appropriately
Act as a trusted contact for clients and families
Provide clear guidance across My Aged Care, Home Care Packages and Support at Home pathways
Maintain respectful and professional communication at all times
Participate in a rotating on-call roster
Promote a positive customer service culture
Uphold Goodwin’s reputation as a quality care provider
Certificate IV or higher in Service Coordination (or extensive experience in similar role)
First Aid Certificate
Current driver’s licence & own vehicle with comprehensive insurance
Strong knowledge of ageing issues, including dementia and behaviour management
Ability to undertake case management to a high standard
Excellent communication, negotiation and problem-solving skills
Strong organisational and time-management capabilities
Competent in Microsoft Office
Experience in aged care, home care services or a related sector
