Job summary

Whiddon is seeking a Community Care Coordinator to lead social and care networks for older Australians in NSW/QLD. You will oversee client care plans, conduct assessments and set goals to promote independent living in the community.

Ideal candidates have leadership experience, knowledge of aged care compliance, and may be RN with AHPRA (not essential). This is a fixed-term, full-time role based in Tamworth, with field visits to meet clients and network.

Job description

We have a large community network of home care employees, home care coordinators and senior community coordinators within our Home Care team, providing domestic, personal care and social support to our clients across NSW and QLD.

The Community Coordinator position is a busy and varied role responsible for building, supporting and strengthening social, family & community networks. You will provide oversight of your clients care plans, undertake assessments and develop goals to assist your clients to live safely & independently in our community. You will be working with a supportive & collaborative team in a role which enables you to work with purpose each and every day, to really make a difference in people lives.

In addition to the above, you will be responsible for:

• Working with the team to deliver high quality services;
• Liaising with other service providers;
• Working with clients to community care programs including Home Care Packages, Commonwealth Home Support Program, NDIS, and ICare;
• Case management;
• Development of Day Programmes
• Supporting staff in their role to deliver and maintain high standards of care

You are a strong leader with previous leadership and people management experience, bring your business acumen to drive results through sales and marketing strategies and most of all have had previous experience in a similar role or within a similar industry so you are across and aware of the complexities of the Aged Care industry, its compliance and clinical aspects of the role.

This will be a fixed-term, full-time position for 9 months, based in our Tamworth office. However, you will be expected to be out and about as part of your role, meeting with clients and attending networking opportunities.

• Qualification(s) and experience in Community Care Coordination or be a Registered Nurse with AHPRA (Not essential)
• Current Class C drivers license
• Current NDIS Worker Check - NDISWC (or willing to obtain)
