Job summary

Goodwin is seeking a compassionate and experienced Registered Nurse to join our Home Care team in Batemans Bay. You will provide clinical care and advice across a diverse range of client needs, including medication management, wound care, gerontology, and palliative care, to help older people remain safe and independent at home.

You will contribute to assessments, care planning and case reviews, working with Home Care Co-ordinators, and supporting transitions from hospital to home with discharge

Qualifications

• Current registration as a Registered Nurse in Australia.
• Nursing qualifications relevant to Home Care/Aged care.
• A current driver’s licence.
• Strong clinical assessment, organisational and problem-solving capabilities.
• Sound knowledge of health, wellbeing and care needs of older people.

Responsibilities

• Deliver high-quality clinical care and nursing support in accordance with client care plans and policies.
• Provide medication, wound and clinical management support to home-based clients.
• Conduct clinical assessments and contribute to individualised care planning.
• Collaborate with Home Care Co-ordinators to respond to changing client needs.
• Coordinate medical, emergency and allied health responses when required.
• Support discharge planning and follow-up for clients transitioning from hospital to home.
• Contribute to behaviour support planning and appropriate strategies.
• Maintain accurate clinical documentation and reporting.
• Participate in quality improvement initiatives and review care policies.

About the role

As a Home Care Registered Nurse, you will provide clinical care and advice across a diverse range of client needs, including medication management, wound care, gerontology, palliative care, dementia support, mental health and transition from hospital to home.

You will also contribute to care assessments, care planning and case management reviews, helping ensure each client's care remains responsive to their individual needs and circumstances.

Key responsibilities include:

• Deliver high-quality clinical care and nursing support in accordance with client care plans, organisational policies and legislative requirements.
• Provide medication, wound and clinical management support to clients living in their own homes.
• Conduct clinical assessments and reviews and contribute to individualised care planning.
• Work collaboratively with Home Care Co-ordinators to identify and respond to clients' changing care and clinical needs.
• Coordinate appropriate medical, emergency and allied health responses when required.
• Support clients transitioning from hospital back into their homes, including discharge planning, clinical assessment and follow-up.
• Contribute to behaviour support planning, assessments and implementation of appropriate strategies.
• Provide clinical expertise across areas including gerontology, palliative care, dementia and mental health.
• Maintain accurate, timely and compliant clinical documentation and reporting.
• Contribute to quality improvement initiatives, including the review of Home Care policies, procedures and clinical practices.
• Maintain ongoing professional development and contemporary clinical knowledge relevant to the care of older people.

About you

You are a confident and compassionate Registered Nurse who enjoys working autonomously while being part of a collaborative multidisciplinary team. You bring sound clinical judgement, strong organisational skills and a genuine commitment to supporting older people to live independently and with dignity.

To be successful, you will have:

• Current registration as a Registered Nurse in Australia.
• Relevant nursing qualifications.
• A current driver's licence.
• Strong clinical assessment, organisational and problem-solving capabilities.
• Sound knowledge of the health, wellbeing and care needs of older people.
• Strong communication and relationship-building skills.
• The ability to work effectively both independently and as part of a team.
• A commitment to delivering respectful, person-centred and culturally responsive care.
• The ability to maintain accurate clinical records and documentation.

Experience delivering nursing services within Home Care or aged care is highly regarded, as are additional qualifications or experience in gerontology, palliative care, dementia, wound management or other relevant clinical specialties.

The successful candidate will be required to maintain all relevant professional registrations, screening clearances and other requirements applicable to working within aged care and/or NDIS services.
