Job summary

genU is seeking a caring and motivated Case Manager to join our Home Care Packages team in Warrnambool, VIC. This full-time role focuses on developing personalised care plans, coordinating services, and ensuring compliant, budget-conscious delivery of supports in clients’ homes.

You’ll work with clients, families and allied health providers to review goals, monitor progress, and adapt plans as needs change.

Qualifications

• Experience in community care and case management is required.
• Understanding of consumer-directed care principles and budgets.
• Knowledge of funding programs and safety standards is preferred.

Responsibilities

• Develop and implement comprehensive, consumer-directed care plans focused on goal outcomes, funding requirements, and measurable results.
• Collaborate with consumers, families, and support networks to ensure care plans reflect individual needs and preferences.
• Conduct regular care plan reviews, monitor progress, and adjust supports in response to changing client needs.
• Coordinate with allied health professionals and service providers while maintaining accurate records and compliance with Home Care Packages guidelines.
