Job summary

Better Health Network is seeking a dedicated Support Worker to assist public housing residents with relocation and settling into new homes, while keeping them connected to their community.

You will provide practical relocation support, develop individual plans, and connect residents with health, housing and social services. Lived experience with relocation is valued, and you will contribute to program improvement and collaborative care.

Qualifications

• Minimum three years’ experience in community, health or social services.
• Certificate IV or Diploma in Community Services or equivalent demonstrated experience.
• Strong communication skills and ability to work with diverse groups.
• Current full or probationary driver’s licence.

Responsibilities

• Building trusted, culturally safe relationships with residents, including people experiencing social isolation.
• Providing practical relocation support, including attending housing viewings and helping residents settle into their new homes.
• Developing individual support plans reflecting residents’ goals and priorities.
• Connecting residents with health, housing, social and community services.
• Supporting access to appointments, community activities and local networks.
• Working collaboratively with families, carers, service providers and the broader team.
• Maintaining accurate client records and contributing to program improvement.
